Well, the time has rapidly approached. I’ve retired from practice, and my office location is closed. This transition remains surreal! Please note the following changes in contact information:

The office number (360.882.1339) will be active through December 2026. My fax number is no longer in service. My only email will be natfammed@gmail.com (NOT health@naturalfamilymedicine.com).

For release of records to your next provider, please complete their form and email the signed copy to natfammed@gmail.com. Alternatively, you may mail it to 2318 SE Bella Vista Road, Vancouver, WA 98683.

Since office is closed, you may make any required payments via this link: https://pay.balancecollect.com/m/nfmcp . My biller provides this link on your statements. If you prefer, you may send a check or your credit card information to 2318 SE Bella Vista Road, Vancouver, WA 98683.

  • Provider options: Here is a list of providers who I recommend and have confirmed are accepting new patients should the need arise while I’m on sabbatical. Please note that most are on insurance plans, but you’d need to check regarding your specific plan for coverage.
  • Dr. Charissa Bausch, ND–360-695-8800 (does not bill insurance)
  • Dr. Julie Mortlock and Dr. Kristin Connolly at NW Health Specialties–360-519-7631
  • Dr. Katherine Zieman (and others!) at Mt Hood Family Medicine and Midwifery–503-218-3100
  • Supplement needs–The in-office medicinary is now closed so please use the following resources until you transition to your next provider. These options should be available through at least December 2026.
  • Fullscript carries most of what I recommend–their link is at the bottom of this email AND on my website.
  • ECS Therapeutics carries the hemp-based CBDa products that I’ve recommended. They can sell directly to you via their website since I’m registered with them for direct patient sales.
  • Herbalist & Alchemist is my preferred botanical source for Hawthorn Solid Extract as well as some other prescribed botanicals. If I’ve prescribed their product(s) for you, you can make online orders with them directly.
  • Wish Garden is another botanical company with products I’ve recommended. Many of their products are available in local health food stores but you can also order via their website.
  • Other botanical tinctures–HerbPharm, Gaia, and Oregon Wild Harvest are excellent over-the-counter sources of botanical formulas.
  • Other bulk herbs and teas–I recommend ordering online with Starwest Botanicals or Mountain Rose Herbs. Most health food stores do not closely monitor the age of their herbs (this includes commercial teas). While they may taste good and have some effect, they may be less potent than what you can find with these companies.
